Alerts and Decision Support

Physician-centric and disease-specific Alert Summary pages give continuously updated dashboard views of each physician´s patient population.

Patient-specific Alerts are displayed on appropriate pages within each patient´s chart, and serve as customizable point-of-care decision support.

The physician-centric Alert Summary page and the disease-specific dashboards include links to lists of all the patients who meet each Alert´s criteria. These lists include patient contact information, which can be displayed and printed, allowing the physician or his or her staff to easily act on these Alerts.

Secure Messaging

The WellCentive Secure Messaging System allows providers and their staff to securely message each other about patient care issues. Patients and their physicians can also securely message each other using this system. Files can be attached and securely sent as well, and the downloading of attached files to the recipient´s computer is encrypted and secure.

The WellCentive Secure Messaging System is also used for customer support questions and concerns, which allows users and WellCentive support staff to securely communicate with each other regarding issues that may occasionally require discussion of patient-specific data.

Medication Reconciliation

The WellCentive system can provide medication reconciliation across the continuum of care - outpatient primary care offices, outpatient specialty care offices, emergency rooms and urgent care centers, hospitals, and home health settings.

Not only can various users of the system view a patient´s Active Medication List, they can also have access to prescription and fill data when the users' e-prescribing system is interfaced with the WellCentive Registry. This can help providers understand who else is prescribing for a given patient and can also help track patient compliance. Did the patient actually fill the prescription for his blood pressure medication? If so, when was it filled?

Through the use of the Patient Report Card, the WellCentive system can also provide medication reconciliation between care providers and the patients themselves. Patients may not be able to understand or provide their medication list, they may forget to refill their medications, or they may misunderstand instructions regarding which medications to start or stop or what doses should be changed, and other issues may arise. Printing the WellCentive Patient Report Card with its Active Medication list for the patient every time a patient is seen can help to improve the communication with patients regarding their medications.

Data Mining

The WellCentive system can be interfaced with any other clinical or administrative patient database in order to capture data and utilize the full features of the WellCentive system, such as Alerts, Reminders, Dashboards, and Reports.

Many EMR´s do not have useful registry features or the ability to perform population management, and interfacing the WellCentive system with an EMR is a cost effective and time sensitive way of providing your patients with high quality preventive care and chronic disease management using the data from your EMR.

Continuity of Care Record

The Web-based nature of the WellCentive system makes it ideal for use throughout a community and across the continuum of care as a Continuity of Care Record.

Different types of health care providers in various settings can access patient data to help them all improve the quality of care that is provided. The data for a given patient is shared and edited by authorized users, which can help to keep the database accurate and up-to-date.

The WellCentive system tracks access to each patient chart, and any access or changes to a chart can be viewed in the Access Log tab in every patient chart.

Patient Portal

The WellCentive system includes a Patient Portal that allows patients to log into the WellCentive system from home and gives them the ability to view, add, and edit some of the data in their charts.

The Patient Portal is an excellent way to address one of the most challenging parts of the Chronic Care Model - Patient Engagement.

De-duping System

Any information system that collects data from disparate sources must successfully prevent the creation of multiple records for a single patient. Unfortunately, there is not yet a universal patient identifier that can be used. It is easy to prevent a second chart for James Smith, DOB = 1-1-1950 from being created if a user or an interface attempts to create one, however what happens if a user or an interface wants to create a chart for Jim Smith, DOB = 1-1-1950, J Smith, DOB = 1-1-1950, or James Smith, DOB = 11-1-1950? Without the proper safeguards in place, there could be several charts for the same patient due to user error or inconsistencies in patient names or other identifiers in different information systems. Duplicate charts would adversely affect the utility of the entire system. The WellCentive De-duping System uses a proprietary algorithm to identify which newly created charts may in fact be duplicate charts for an existing patient. The WellCentive De-duping System can also examine the entire database on demand to identify any other potential duplicates. The system then provides the user with a user interface that allows them to block the creation of duplicate charts or merge existing duplicate charts into a single chart.

WellCentive specializes in the collection and aggregation of clinical and administrative patient data from disparate sources.
