Chronic Disease Management

The WellCentive system can interface with in-home medical devices such as glucometers, scales, blood pressure devices, medication dispensers, etc. Home care nurses can post digital photos of wounds to any patient´s chart and discuss the patient´s care with the treating physician using the WellCentive Secure Messaging system.

Outbound interfaces to a variety of payers is a cost effective and efficient way of sending payers the necessary PFP data on a regular basis. The payers are supportive, because these interfaces provide them with more complete and more accurate data more frequently and lower their costs as well.

Population Management

Physician Organizations that do not use the Messenger Model for contracting must be Clinically Integrated according to the Federal Trade Commission. PHO´s and IPA´s that are in violation of this federal law can be shut down, their physicians can be fined, and their officers can be incarcerated.

Clinical Integration means that a physician organization´s primary care and specialty physicians are actively working together on an ongoing basis to control health care costs and to improve clinical quality.

Direct Reporting to Payers

WellCentive was chosen as one of twelve registry vendors systems across the United States to test direct reporting of clinical and administrative data to CMS for the PQRI program.
